Click on the link below to open and print our CME Preapproval form or Post CME Submission form for registering your CME activity. Mail it or fax it to us with a check or credit card payment. If you wish to pay online using Paypal, you can do so at our Payment Center. You will need the Payment confirmation number to add to the CME forms.
1) CME pre-approval form: Please fax this form to 1-(610) 870-3720. You can include your payment information on it, or include a check. Or you can choose to pay it online at the payment center and add the Paypal confirmation number. You can also alert us about the fax by using our contact form. If you do not have access to fax, you can submit the same information using the contact form too.
2) CMETRIP.org Instructions: Do you want to print out our easy 4 step instructions? Then here is the pdf file for it.
3) Late CME Submission: Have you already completed your CME within last two months? Then you should use this form to submit your request for registration. Do not forget to attach all your CME Certificates. There is an additional $100/- processing fee.
